Laboratory Closeout/Relocation

Moving a laboratory is a complex process with many items that must be addressed in order to fully closeout the space being vacated.

Laboratory/Research groups leaving the University or relocating to a new location on campus are required to complete the EHS laboratory closeout/relocation process to ensure that the lab is properly closed out and potential hazards are not left behind.

The Closeout/Relocation process is tracked in the Safety Administrative Management (SAM) system.

To begin the closeout process please notify EHS at SAM@ehs.utah.edu.

Once notification has been made using the email address above the lab contact will receive an email asking them to complete a simple 2 minute questionnaire.  The information from the questionnaire will be used to assign a closeout/relocation self checklist that will assist lab personnel with completing all required closeout activities.

Once the closeout/relocation checklist is complete and submitted, the system will assign a final walk-through inspection with an EHS specialist to verify that all closeout steps have been completed.
