Injury or Illness

  • Get Medical Attention
  • Do not disturb the accident site until it has been investigated and released. Tools, equipment, materials or other evidence that might pertain to the cause of such accident may not be removed or destroyed until so authorized by the Labor Commission or one of its Compliance Officers.
  • Do the paperwork (See below).

  • Within 72 hours (or as soon as possible) following the incident, complete the Workers Compensation first report of injury form 122 (also called an E-1)

  • If the accident is “serious” (see definition below) or involves a fatality IMMEDIATELY notify Occupational and Environmental Health and Safety (OEHS), 24 hours a day, 7 days a week.During work hours, call 801-581-6590. After hours, call University Police Dispatch, 801-585-2677.  OEHS will notify Utah Occupational Safety and Health Division within the mandated 8-hour reporting time frame.

    Disabling and serious include, but are not limited to the following: any injury or illness resulting in admittance to the hospital, and/or injuries such as amputation, fracture, deep cuts, severe burns, electric shock, sight impairment, loss of consciousness, and concussions.

Student or Visitor

Fill out the Risk Management University of Utah Injury/Accident report form. Return it to Risk Management.
